Commitment to Serve Form

If you have been nominated for an LEC Officer position and wish to be a candidate, you must contact the AFA-CWA Membership Services Department and state your commitment to serve. This can be done by completing the form below. Once we receive this information, a member of the AFA-CWA Membership Services Department will contact you about being a candidate. Complete this form only if you wish to serve as an LEC Officer for the 3 year term.
